Tax Documents

A paper copy of the IRS Form 1095-C will be mailed to you. This form provides information about your health coverage and is not a W-2 (income) form. Please note that you do not need to submit your 1095-C form when filing your taxes; it is provided for your information and records only. In addition to the copy mailed to you, you can access your 1095-C electronically through PlanSource.

When the 1095-C form becomes available online, a notification email will be sent to the email address on file in PlanSource. Follow these three steps to download your form:

  1. Log in to PlanSource.
  2. Select ACA Documents under the Documents tab.
  3. Within that tab, click on List Of Your Documents By Year to view your latest 1095-C form.

If you are a UC Irvine resident or fellow, or if you cannot access PlanSource, please contact your GME plan coordinator for assistance with obtaining your 1095-C form.

COBRA participants and terminated residents will receive their 1095-C form by mail.

Update your contact information (specifically your email and mailing address) in PlanSource to make sure that we have the most up-to-date information for you.
